The United States Coast Guard's National Maritime Center (NMC) has released updated CG-719 series forms with an expiration date of April 30, 2026

You can easily verify this by checking the upper right corner of the forms.

Can I still use the CG-719 series forms that expired in 2021?

The previous CG-719 forms display an expiration date of March 31, 2021 in the upper right corner. While these forms were once valid, we strongly recommend transitioning to the current versions.
