Recommendation: Download the FCC 605 Main Form to your desktop before filling it out. If the form opens in your web browser, save it to your computer first, then open the saved file from your desktop. This helps ensure your information is entered and saved correctly.

The 605 Main Form is used for all FCC credentials, including both initial applications and license modifications.

Fill out the form to the best of your ability. Since this form is used for various FCC licenses, not all sections apply to your application. The fillable form highlights the key components we need. Simply complete these highlighted sections and leave the rest blank if they don’t apply. Â

For original applications, select NE (New) - this applies even if you are upgrading from one license level to another. If you are modifying your current license by adding a Ship Radar Endorsement, select MD (Modification).Â

An FRN is a number issued by the FCC as your registration identity throughout your journey with the FCC. Each FRN is unique to the registrant. If you do not have an FRN, please register for one here.Â

If you marked “Yes” for #28 on the FCC 605 Main Form, you would also need to check off “Yes” on #8 of the same form. Â
