Recommendation: Download the FCC 605 Schedule E to your desktop before filling it out. If the form opens in your web browser, save it to your computer first, then open the saved file from your desktop. This helps ensure your information is entered and saved correctly.
The FCC 605 Schedule E Form is required for all FCC credentials, including both first-time applications and license modifications. Page 1 contains instructions, while page 2 includes the sections we need you to complete.
Only the completed sections on page 2 should be submitted back to us; please do not send the instruction pages.
Complete the form to the best of your ability. The FCC 605 Schedule E is used for various licenses, so not all sections are required for your application. The fillable form will highlight the key sections that need to be completed. Please focus only on those sections when filling out the form.
For #1, you need to specify which FCC license you are applying for. We only offer the options listed in the left-hand column. Make sure to check only one box that corresponds to the license you are seeking.

Here is a breakdown of the exam numbers and which licenses you can obtain with them:
You will leave the Endorsement Type blank UNLESS you are requesting a Ship Radar Endorsement. If you are selecting a Ship Radar Endorsement, please select the corresponding box.
To obtain an FCC license, you must be eligible to work in the United States. Therefore, in the certification section of your FCC form, you must select YES for the question "Are you eligible for employment in the United States?" Additionally, be sure to include your birthdate.
Finally, when completing the certification category section, select the first option out of the three available categories. This is a required step for your application.
