Keeping your application materials up-to-date is essential for a smooth credentialing process.
Here’s what you need to know about the current CG-719 series forms:
The United States Coast Guard's National Maritime Center (NMC) has released updated CG-719 series forms with an expiration date of September 30, 2028.
You can easily verify this by checking the upper-right corner of the forms.
💡 TIP! You can find the most recent application forms on our website here.
The previous CG-719 forms display an expiration date of April 30, 2026. While these are valid up to this date, you must submit your entire application packet by this date to keep them valid OR use the new forms if you plan on submitting after April 30, 2026.
The prior forms displayed March 31, 2021 in the upper right corner. While these forms were once valid, you must use the newest forms.
If you’ve already completed the older forms, it’s essential to contact the National Maritime Center (NMC) to confirm whether they will accept them. Using the most up-to-date forms minimizes the risk of delays or complications with your application.
Staying informed and prepared with the correct documentation ensures your credentialing process proceeds efficiently and without unnecessary setbacks.
💡 TIP! We recommend using the current forms with the 2028 expiration date. If you've completed older forms with the 2026 or 2021 expiration date, please contact the National Maritime Center to confirm their acceptance.
